Organ by MadSoul
This is a mini-theme by one of my good friends from back in the PSP scene, madsoul. He's worked very hard on it and personally I think it rocks.

For those who don't know, the files go in:
music player goes in/System/Library/Frameworks/MoviePlayerUI
the sliders go in/System/Library/Frameworks/TelephonyUI.framework

Wow this is very sweet, I'm a bit hesitant of adding customize though, had some lag issues with it. Once it's out beta, perhaps.
